Support coordination helps a person understand their plan, connect with suitable services and build confidence in managing supports. A coordinator should work with the person’s goals, choices and communication needs rather than taking over decisions.

What this support may include

Understanding your plan and budget; identifying local and specialist providers; arranging service agreements; preparing for plan reviews; building skills to coordinate supports independently.

Finding the right fit

Start with the person’s goals, location, funding and practical preferences. Explain the support needed, the days and times that matter, and any accessibility, language or communication requirements. Ask more than one provider when possible so you can compare capacity, approach and costs.

Questions to ask a provider

Do they understand your funding type? How will they keep you informed? Can they support your preferred communication style? What happens if your coordinator is away?

Funding and next steps

Funding arrangements differ. If you use NDIS funding, confirm whether the provider can work with your plan-managed, self-managed or agency-managed arrangement. If you are unsure, include what you know in your request and ask the provider to explain their options clearly.
