Start with consent
Confirm what the participant agrees can be shared, who may be contacted, and their preferred communication approach before making a request.
Use clear requirements
Include the service required, area, funding, timeframes, accessibility needs and non-negotiables. Do not include unnecessary health or identity information in the initial form.
Keep choice central
A referral is not a decision. Give the participant enough information and time to compare options, ask questions and choose whether to proceed.
Keep requests current
Update or close requests that are no longer needed. This protects provider time and makes available capacity more visible.
