What we collect

For support requests, this can include your name, contact details, preferred contact method, location, the kind of support you are looking for and any practical preferences you choose to share. For provider profiles, this includes business and work-contact details.

How to use the forms safely

Please do not enter NDIS numbers, date of birth, diagnoses, medical reports or detailed health information in Care4AU forms. You can discuss private details directly with a provider after you choose to contact them.

How requests are handled

Care4AU uses request details to respond to you and help identify relevant services. We will discuss sharing a request with a provider before doing so. Provider profile information is used to run the directory and communicate about the profile.

Questions or corrections

Use the Care4AU contact page for a question about the information you submitted, or report a concern if a provider profile appears inaccurate.
